Description
The Cardiac Care Bridge program was a randomized nurse-coordinated transitional care program
combining case management, disease management and home-based rehabilitation for
hospitalized frail older cardiac patients. This qualitative study explored the
experiences of patients’ participating in this nurse-coordinated transitional
care intervention, as part of a larger process evaluation, in order to support
interpretation of the neutral trial outcomes and the contribution of the
intervention components from participants’ point of view.The data of this study consists of the MAXQDA-file in which the transcripts and the coding process are included.
