Description

The Cardiac Care Bridge (CCB) transitional care programme evaluated the impact of combining case management, disease management and home-based cardiac rehabilitation (CR) on hospital readmission and mortality. The effectiveness of the CCB program was evaluated in a multicenter randomized clinical trial that was conducted between June 2017 and March 2019 in six hospitals in the region of Amsterdam. In total, 306 patients > 70 year at high risk of readmission and mortality participated in this study. The primary outcome was a composite of readmission and mortality within six months
