Abstract
Het lectoraat Transmurale Ouderenzorg is onderdeel van het kenniscentrum ACHIEVE van de faculteit Gezondheid. ACHIEVE heeft ‘Complex Care’ als focus. De complexiteit van de acute zorg voor oudere patiënten, waarin samenwerking en coördinatie van essentieel belang zijn, sluit hier naadloos op aan.
